Ask your physician to prescribe either a compounded T4 and T3 or the combination of Synthroid (or Levothyroxine) with Cytomel. Synthroid is T4 and Cytomel is T3 and are really the same as the cmpd T4 and T3. Just because the day you had the last TSH blood test it was 3.0 does not mean that it's consistently this amount. With Hashi's there is thyroid inflammation and when there's thyroid inflammation there will be some thyroid cells which are being broken apart. This releases some thyroid hormones and likely during this time you'll feel better. And when these thyroid hormones are released, when combined with the thyroid medication you are taking, your TSH will likely go down to even lower than 3.0. This temporarily decreases TSH and thyroid inflammation goes down. And when your TSH goes down then your thyroid's production goes down as well and now you are feeling tired again. The approach is to keep TSH low, which is very important to keep thyroid inflammation down. This requires slightly increasing your thyroid medication. You may have to do this increase a few times until TSH finally comes to below 1.0. Then thyroid inflammation will recede and you'll reach a more steady state of thyroid hormones, primarily from the thyroid prescription. Levo is also only one of the thyroid hormones, thyroxine or T4, and it doesn't really have too much to do with energy production of clarity of thought. It must be converted to the thyroid hormone T3 which is the hormone which does activate metabolism. This conversion is done primarily within your liver and other parts of the body. Many people do not convert T4 to T3 all that well. Selenium helps with this conversion as does the hormone produced by the adrenals called cortisol. Too much cortisol is a problem but most people with Hashi's have too little and therefore suffer from low thyroid hormone symptoms. Optimizing cortisol levels is a very important part of improving thyroid hormone levels. You also have the option, if there is poor conversion, to get a prescription for T3 called Cytomel which can be added on to your Levo. I suggest getting a lab test that includes TSH, T4, Free T4 and Free T3. This way you can tell if you are converting well or not. My TSH was 64.99 and now is down to 3.0 and although i feel much better i am still having some really bad days. it is like the medication (levo)just is not working some days. even on good days i get really cold and hot. i mean freezing cold. sometime i am cold but hot and sweaty on the surface. the bad days i am hot and cold and cant remember what i am doing half the time.everyone points me to sites and i look if i remember(usually forget with in 30 secs)- i have the attention span of a gnat. is there any hope? hi, recently diagnosed with hashimotos.
